About The Position

Luminos Hospice is seeking a skilled and compassionate On-Call LPN to provide quality care to patients during weekends. The nurse administers nursing care to patients of all ages in their places of residence and coordinates care with the Interdisciplinary Group/Team, patients, families, and referring agencies. The role assumes responsibility for and participates in the coordination of care. The schedule is Friday at 5 p.m. through Monday at 8 a.m.

Requirements

• Current state LPN license.
• Current driver's license.
• Current CPR certification.
• Two years of experience as a nurse in a clinical care setting.
• Nursing skills consistent with generally accepted standards of practice.
• Knowledge of state and federal regulations.
• Ability to work independently and make sound decisions.
• Strong attention to detail and organizational habits.
• Excellent verbal and written communication skills.
• Good interpersonal skills.
• Reliable transportation and valid auto liability insurance.

Nice To Haves

• One year of full-time hospice experience preferred.
