About The Position

Concierge Home Care is seeking an experienced Licensed Practical Nurse (LPN) to join its remote Virtual Care team. This Florida-based, work-from-home role supports high-risk patients in remaining safely at home through proactive remote care, clinical assessment, and intervention. The position is scheduled every Friday, Saturday, and Sunday from 8:00 AM to 8:00 PM, with compensation of $21.00–$23.00 per hour. Employment is contingent upon successful completion of an AHCA Level 2 background screening.

Requirements

• Reside in Florida; the position is 100% work from home and Florida residency is required due to operational, licensing, and/or business requirements.
• Hold an active, unencumbered Florida LPN license.
• Have at least 2 years of home health experience.
• Have experience with triage and hospitalization prevention.
• Have strong clinical judgment and communication skills.
• Be comfortable using telehealth platforms and electronic medical record (EMR) systems.
• Have high-speed internet and a HIPAA-compliant, distraction-free workplace.
• Be computer savvy and able to use Teams, Outlook, and Excel.
• Successfully complete an AHCA Level 2 background screening.

Responsibilities

• Conduct virtual patient visits and clinical assessments.
• Perform triage and identify early warning signs or risk trends.
• Support chronic disease management and provide patient education.
• Monitor telehealth data and escalate concerns appropriately.
• Help prevent avoidable emergency room visits and rehospitalizations.
• Collaborate with interdisciplinary care teams.
• Document patient interactions accurately and promptly.
• Participate in care planning and performance improvement initiatives.
• Support the development of virtual care best practices.
