About The Position

The Clinical Care Manager serves as the clinical point of contact for a caseload of home care clients, their families, and assigned caregivers. The position is intended for a licensed RN who can move between clinical assessment, conflict resolution, and administrative problem-solving. The schedule is weekends and holidays. The role is primarily remote, with potential for occasional in-home visits, and requires availability to respond to urgent client or caregiver issues after normal business hours. It is a fast-paced environment with frequent interruptions and shifting priorities.

Requirements

  • Active, unencumbered RN license in the state of practice.
  • Minimum of 2–3 years of clinical nursing experience.
  • Working knowledge of common geriatric and chronic conditions, including hypertension, UTIs, altered mental status, and skin or wound issues.
  • Strong triage and clinical decision-making skills, including sound judgment about when to escalate.
  • Demonstrated ability to de-escalate emotionally charged situations involving patients, families, and staff.
  • Ability to manage multiple clinical, interpersonal, and logistical issues, follow-ups, and documentation requirements.
  • Strong empathetic and professional communication, conflict-resolution, organizational, and problem-solving skills.
  • Comfort learning and troubleshooting scheduling or EVV platforms and supporting caregivers with system issues.

Nice To Haves

  • Home health, hospice, or care management experience.
  • Experience with electronic visit verification (EVV) or care management software such as AlayaCare.

  • Serve as the first point of contact for clinical calls, triage symptoms, and determine appropriate urgency and response.
  • Evaluate and respond to reported changes in client condition, including vital-sign abnormalities, altered mental status, suspected infections, rashes, and other new or worsening symptoms.
  • Provide guidance following incidents such as patient falls, including assessment recommendations, escalation to EMS or a physician when warranted, and documentation.
  • Answer medical questions from patients and families within nursing scope and escalate issues to an appropriate provider when necessary.
  • Use nursing judgment to evaluate medication administration decisions against vital signs and prescriber-established parameters, and determine next steps when doses are held or missed.
  • Ensure medication administration or assistance by home health aides and CNAs complies with applicable Florida laws and regulations, including requirements for delegation, training, competency validation, and restrictions on controlled substances.
  • Conduct or oversee required RN medication reviews and reconciliation before aides or CNAs assist with routine self-administered medications, and ensure written consent is on file.
  • Respond to antibiotic administration questions, including timing, missed doses, adverse reactions, and lack of therapeutic response, escalating to the prescriber as needed.
  • Provide case oversight for medication concerns such as polypharmacy, refill or access issues, and confusion about medication regimens.
  • Coordinate with providers, dialysis centers, outside agencies, and external stakeholders regarding appointments, treatments, care plans, and authorizations.
  • Address caregiver performance concerns and help resolve issues involving duties, patient supervision, transportation, or short-notice call-outs.
  • Help manage last-minute caregiver call-offs by identifying and dispatching replacement coverage when needed.
  • Investigate and mediate client-caregiver compatibility conflicts, and support transitions when a client requests a caregiver change.
  • Support caregivers with scheduling and care-delivery challenges, including animal-related concerns in the home.
  • Train and support caregivers in using AlayaCare or a comparable EVV or scheduling system, and troubleshoot access and documentation issues.
  • De-escalate difficult interactions with patients and families and use empathetic, professional communication with patients who are resistant to care.
  • Manage family expectations and maintain clear, timely communication with clients, families, caregivers, providers, and internal staff.
  • Document clinical assessments, incidents, communications, and care plan changes accurately and promptly in accordance with agency policy and regulations.
