About The Position
Elara Caring is seeking an After-Hours/On Call Registered Nurse to provide compassionate hospice care. The role includes assessing patients and families, monitoring patient condition, supporting implementation of the interdisciplinary Plan of Care, educating care staff on hospice philosophy and symptom management, and providing support at the time of death. The schedule is Saturday and Sunday, 12-hour shifts, plus one day of the week of the employee’s choice.
Requirements
- Current state license as a Registered Nurse.
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
- At least 1 year of experience as a nurse in a clinical care setting.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
- Valid driver’s license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

- Experience in a hospice or home health environment.
Responsibilities
- Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
- Monitor patient condition and communicate significant changes, concerns, or safety issues to the physician and care team.
- Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
- Educate staff in nursing homes, hospitals, assisted living facilities, and other care settings on hospice philosophy, pain management, and symptom management.
- Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
