FT Evenings Hospice Visit Nurse (RN)
About The Position
The Visit Registered Nurse assesses and identifies patient and family needs, then plans, implements, evaluates, and documents individualized hospice nursing care in accordance with the interdisciplinary plan of care and the policies and expectations of the Clinical Services Department. The nurse coordinates care and collaborates with the Interdisciplinary Group and other medical providers, and supervises hospice care provided by Licensed Practical Nurses and Certified Nursing Assistants. Assigned patients may be in a home, skilled nursing facility, assisted living facility, or acute care setting. The nurse may visit home patients to meet team nurse coverage needs, make unplanned visits, follow up on new admissions, conduct admission visits for medically eligible patients entering Hosparus Health care, or conduct facility-to-home admissions. The Visit Registered Nurse demonstrates excellence in nursing practice and supports the organization’s needs in providing nursing care and services to all Hosparus Health patients. Schedule: Sunday, Tuesday, Wednesday, and Thursday, 5:00 PM to 1:30 AM, with on-call requirements. Service area: Louisville and surrounding communities.
Requirements
- Current RN licensure in Kentucky and/or Indiana as required by the designated position, or ability to obtain required license(s) upon offer.
- Valid driver’s license.
Nice To Haves
- Bachelor of Science in Nursing (BSN) desired.
- Certification in hospice/palliative care preferred.
- One year of nursing experience preferred.
- Knowledge of palliative care helpful.
- Experience using technology-based tools, including an electronic medical record, strongly preferred.
Responsibilities
- Assess and identify patient and family needs.

- Provide nursing care and services to Hosparus Health patients while demonstrating excellence in nursing practice.
