Hospice Visit Nurse (RN)
About The Position
The Visit Registered Nurse assesses and identifies patient and family needs, then plans, implements, evaluates, and documents individualized hospice nursing care in accordance with the interdisciplinary plan of care and Clinical Services Department policies and expectations. The nurse coordinates and collaborates with the Interdisciplinary Group and other medical providers, and supervises hospice care provided by Licensed Practical Nurses and Certified Nursing Assistants. Patients may be located in homes, skilled nursing facilities, assisted living facilities, or acute care settings. Visits may include coverage for team nurses, unplanned visits, follow-up after new admissions, admission visits for medically eligible patients, and facility-to-home admissions. The Visit Registered Nurse demonstrates excellence in nursing practice and supports the organization’s nursing care and service needs. Schedule: Monday through Friday, 8:30 a.m. to 5:00 p.m., with on-call requirements. Service area: Louisville and surrounding communities.
Requirements
- Current RN licensure in Kentucky and/or Indiana as required by the designated position, or ability to obtain the required license(s) upon offer.
- Valid driver's license.
Nice To Haves
- Bachelor of Science in Nursing (BSN) desired.
- Certification in hospice/palliative care preferred.
- One year of nursing experience preferred.
- Knowledge of palliative care helpful.
- Experience using technology-based tools, including an electronic medical record, strongly preferred.
Responsibilities

- Demonstrate excellence in nursing practice and support nursing care and services for Hosparus Health patients.
