About The Position
Care Initiatives Hospice is seeking a Registered Nurse to provide compassionate, comprehensive hospice care to patients with life-limiting illnesses. The nurse meets patients and families in their preferred settings, including private homes, long-term care communities, assisted living residences, and hospitals. Care takes a holistic approach to patients’ physical, emotional, and spiritual needs and is delivered collaboratively with family members, caregivers, and the interdisciplinary team, with dignity and respect for patients’ needs and wishes.
Requirements
- Active Iowa RN license.
- Active CPR certification.
- Hospice or palliative care experience.
- Current driver’s license and automobile liability insurance.
- Ability to travel between multiple locations within the assigned region.
- Home access to the internet.
- Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment.
Nice To Haves
- Two years of verifiable experience working as a nurse (RN/LPN) is preferred.
Responsibilities
- Use clinical nursing knowledge, physical assessment, and procedural skills to provide high-quality patient care in the patient’s place of residence.
- Develop and oversee the plan of care under physician direction and ensure appropriate interdisciplinary involvement.
- Perform initial and comprehensive patient assessments in accordance with policies and procedures.
- Conduct ongoing patient visits and interdisciplinary assessments.
- Provide quality care and complete clear, concise, and accurate clinical documentation.
- Provide patient, caregiver, and family counseling and education as indicated in the plan of care.
- Develop and continually update plans of care identifying the services required to meet each patient’s needs.
