About The Position

The On Call Hospice Visit Nurse provides compassionate on-call care to hospice patients, responding to urgent needs with empathy and professionalism. The nurse collaborates with the RN Case Manager and Interdisciplinary Team (IDT) to implement tailored interventions and support a comprehensive, responsive plan of care. The role includes assessing and meeting the multifaceted needs of patients and families under the supervision of a Registered Nurse, maintaining thorough documentation, and delivering continuous care in accordance with hospice standards and protocols. This full-time position serves Morris and Sussex Counties and is scheduled Monday–Friday, 5 p.m.–8 a.m., or Friday–Monday, 5 p.m.–8 a.m. The role offers independence supported by leadership and a collaborative, team-oriented work culture.

Requirements
