About The Position

The Visit Registered Nurse assesses and identifies patient and family needs and plans, implements, evaluates, and documents individualized hospice nursing care in accordance with the interdisciplinary plan of care and the Clinical Services Department’s policies and expectations. The nurse coordinates care and collaborates with the Interdisciplinary Group and other medical providers, and supervises hospice care provided by Licensed Practical Nurses and Certified Nursing Assistants.

Patients may be assigned in homes, skilled nursing facilities, assisted living facilities, or acute care settings. Visits may include covering home patients based on team needs, making unplanned visits, following up on new admissions, conducting admission visits for medically eligible patients, and conducting facility-to-home admissions. The nurse demonstrates excellence in nursing practice and supports the organization’s provision of nursing care and services to all patients.

The schedule is Monday through Friday, 8:30 AM to 5:00 PM, with on-call requirements. The service area is Louisville and surrounding communities.

Requirements

• Current RN licensure in Kentucky and/or Indiana as required by the designated position, or ability to obtain required license(s) upon offer.
• Valid driver’s license.

Nice To Haves

• Bachelor of Science in Nursing (BSN) desired.
• Certification in hospice or palliative care preferred.
• One year of nursing experience preferred.
• Knowledge of palliative care is helpful.
• Experience using technology-based tools, including an electronic medical record, is strongly preferred.

Responsibilities
