About The Position

Care Initiatives Hospice is hiring a Registered Nurse to provide compassionate, comprehensive hospice care to patients with life-limiting illnesses. The nurse meets patients and families in their preferred settings, including homes, long-term care communities, assisted living residences, and hospitals. Care is holistic, addressing patients’ physical, emotional, and spiritual needs, and is delivered in collaboration with family members and caregivers to honor patients’ needs and wishes.

Requirements

• Active Iowa RN license.
• Active CPR certification.
• Hospice or palliative care experience.
• Current driver’s license and automobile liability insurance, and ability to travel between multiple locations within the region.
• Home access to the internet.
• Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment.

Nice To Haves

• Two years of verifiable experience working as a nurse (RN or LPN) is preferred.

Responsibilities

• Use clinical nursing knowledge, physical assessment, and procedural skills to deliver high-quality patient care in the patient’s place of residence.
• Develop and oversee the plan of care under physician direction, ensuring appropriate interdisciplinary involvement.
• Perform initial and comprehensive patient assessments in accordance with policies and procedures.
• Provide ongoing visits and interdisciplinary assessments of patients.
• Deliver quality care and complete clear, concise, and accurate clinical documentation.
• Provide patient, caregiver, and family counseling and education as indicated in the plan of care.
• Develop and continually update plans of care to identify the care and services needed to meet patient-specific needs.
