FT Hospice Visit Nurse (RN)
About The Position
The Visit Registered Nurse assesses and identifies patient and family needs, then plans, implements, evaluates, and documents individualized hospice nursing care in accordance with the interdisciplinary plan of care and Clinical Services Department policies and expectations. The nurse coordinates care and collaborates with the Interdisciplinary Group and other medical providers, and supervises hospice care provided by Licensed Practical Nurses and Certified Nursing Assistants. Patients may be assigned in homes, skilled nursing facilities, assisted living facilities, or acute care settings. Visits may include coverage for home patients, unplanned visits, follow-up after new admissions, admission visits for medically eligible patients, and facility-to-home admissions. The Visit Registered Nurse demonstrates excellence in nursing practice and supports the organization’s needs in providing nursing care and services to all Hosparus Health patients. The schedule is Monday through Friday, 8:30 AM to 5:00 PM, with on-call requirements. The service area includes Louisville and surrounding communities.
Requirements
- Current RN licensure in Kentucky and/or Indiana as required by the designated position, or ability to obtain required license(s) upon offer.
- Valid driver’s license.
Nice To Haves
- BSN desired.
- Certification in hospice or palliative care preferred.
- One year of nursing experience preferred.
- Knowledge of palliative care helpful.
- Experience using technology-based tools, including an electronic medical record, strongly preferred.
Responsibilities

- Visit assigned patients in homes, skilled nursing facilities, assisted living facilities, or acute care settings.
- Make home visits for team nurse coverage needs, unplanned visits, and follow-up on new admissions as needed.
- Conduct admission visits for medically eligible patients and facility-to-home admissions as needed.
- Provide nursing care and services to Hosparus Health patients and demonstrate excellence in nursing practice.
