About The Position

The Visit Nurse provides high-quality, patient-centered direct nursing care to individuals facing life-limiting illnesses. The role includes scheduled and unscheduled visits to patients in their homes or residential facilities to support comfort, dignity, and the needs of patients and their families. The position is based at the Main Office in Eureka, California, and requires daily local driving. It is a per diem, day-shift position requiring at least two shifts per month, including weekends.

Requirements

• Graduate of an accredited nursing program.
• Current California Registered Nurse license.
• At least one year of experience as a professional nurse within the previous three years, or equivalent experience in a BSN program.
• Strong clinical assessment skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Ability to manage a caseload effectively and efficiently.
• Compassionate, patient-centered approach to care.
• Proficiency with electronic medical records (EMR) systems.
• Valid driver's license and reliable transportation.
• Must be able to work two shifts per month, including weekends.

Nice To Haves

• Experience in hospice or home health.
• Experience working effectively with an interdisciplinary team.
• CHPN certification.

Responsibilities

• Conduct in-home visits to assess patient condition, manage symptoms, and provide emotional support.
• Administer prescribed medications and treatments, including pain and symptom management.
• Collaborate with interdisciplinary team members, including case manager RNs, physicians, social workers, chaplains, and aides.
• Educate patients and families about disease progression, care plans, and available resources.
• Document care provided and communicate updates to the hospice team.
• Respond to urgent patient needs and provide crisis intervention when necessary.
• Ensure compliance with hospice policies, procedures, and regulatory standards.
