About The Position

Trellis Supportive Care is hiring a full-time Hospice Registered Nurse to provide compassionate nursing care to homecare patients and their families throughout the service area. The role delivers patient-centered care in patients’ homes, long-term care facilities, and assisted living facilities, with flexibility to manage patient visits from home. The nurse collaborates with interdisciplinary homecare teams and clinical leadership to meet patients’ needs and support families.

Requirements

• Current North Carolina Registered Nurse (RN) license.
• Strong nursing assessment and critical thinking skills.
• Excellent organizational and problem-solving abilities.
• Exceptional customer service and communication skills.
• Ability to foster a positive culture built on respect, compassion, and teamwork.
• Experience with electronic medical records.

Nice To Haves

• Axxess EMR experience.

Responsibilities

• Provide direct and indirect nursing care to homecare patients and families throughout the service area.
• Deliver nursing services, including assessments, symptom management, preventative care, education, and time-of-death care, according to the plan of care.
• Collaborate with homecare teams and make daily visits to patient homes, long-term care facilities, and assisted living facilities.
• Complete assigned daily visits, as determined by clinical leadership and patient acuity.
• Communicate promptly and appropriately with physicians and other clinicians to support patient and family needs.
• Maintain accurate and timely clinical documentation and physician orders in the Electronic Medical Records (EMR) system.
