About The Position

Community Home Health Care is hiring registered nurses and licensed practical nurses to provide patient care visits in clients’ homes. The role involves delivering one-on-one care that helps clients feel supported and safer at home. Part-time and PRN opportunities are available, and full-time opportunities may also be considered. Visits may be scheduled early in the morning, in the evening, or on weekends.

Requirements

• Hold a current Indiana RN or LPN license.
• Be compassionate and dependable.
• Be comfortable making home visits.
• Communicate clearly with patients, families, and coworkers.
• Be able to work early morning, evening, or weekend hours.

Nice To Haves

• Home health experience or interest in providing care in a home-based setting.

Responsibilities

• Provide care according to the patient’s care plan, provider orders, and the nurse’s license.
• Monitor patients and report changes in condition.
• Complete ordered nursing treatments and wound care within the nurse’s scope of practice.
• Support medication-related care as directed.
• Teach patients and caregivers about care routines and safety.
• Document visits and communicate with the care team.
