About The Position

CareSense is seeking Registered Nurses to conduct home visits throughout the Philadelphia area. The role involves assessing patients in their homes, performing supervisory visits, and completing 485/Plans of Care. Requirements

Currently hold an RN license in Pennsylvania.
Have a valid state driver’s license and car insurance.

Responsibilities

Visit clients in their residences within designated geographic territories.
Schedule and perform monthly supervisory visits.
Complete timely and accurate clinical notes, including documentation of patient progress.
Evaluate client status and workers’ skills while the staff member is present.
Document communications with clients, family members, and support staff.
Provide patients, families, and caregivers with education and information pertinent to diagnoses and safety.
Counsel and educate patients and families about meeting healthcare and related needs.
Write 485/Plans of Care for patients.
Perform other projects and duties as assigned.
