About The Position

The Visit Registered Nurse assesses and identifies patient and family needs, then plans, implements, evaluates, and documents individualized hospice nursing care in accordance with the interdisciplinary plan of care and Clinical Services Department policies and expectations. The nurse coordinates and collaborates with the Interdisciplinary Group and other medical providers, and supervises hospice care provided by Licensed Practical Nurses and Certified Nursing Assistants.

Patients may be assigned in homes, skilled nursing facilities, assisted living facilities, or acute care settings. Depending on team coverage and patient needs, the nurse may visit home patients, make unplanned or follow-up visits for new admissions, conduct admission visits for medically eligible patients entering Hosparus Health care, or conduct facility-to-home admissions. The nurse demonstrates excellence in nursing practice and supports the organization’s nursing care and services for all Hosparus Health patients.

Schedule: Saturday and Sunday, 8:30 AM to 9:00 PM; plus one shift during the week, with on-call requirements. Service area: Louisville and surrounding communities.

Requirements

• Current RN licensure in Kentucky and/or Indiana, as required by the designated position, or ability to obtain required license(s) upon offer.
• Valid driver’s license.

Nice To Haves

• BSN desired.
• Certification in hospice/palliative care preferred.
• One year of nursing experience preferred.
• Knowledge of palliative care helpful.
• Experience using technology-based tools, including an electronic medical record, strongly preferred.

Responsibilities

• Assess and identify patient and family needs.

• Make home visits based on team nurse coverage needs, unplanned needs, or follow-up needs for new admissions.
• Conduct admission visits for medically eligible patients entering Hosparus Health care and facility-to-home admissions as needed.
• Demonstrate excellence in nursing practice and support the organization’s provision of nursing care and services to patients.
