About The Position

Nemours Children's Health is seeking an on-site Virtual Care Registered Nurse in Jacksonville, Florida, supporting Advanced Care at Home and other clinical care programs. The Virtual Care RN partners with bedside nursing teams to provide professional nursing care for patients in acute care, critical care, emergency, home-based care, and remote patient monitoring settings. Using phone, two-way video, remote patient monitoring, and other technologies, the nurse provides assessments and interventions, patient and family education, and discharge planning. The nurse applies the nursing process from admission through discharge, using clinical judgment to assess complex patient information, plan and coordinate care, monitor outcomes, and adapt interventions to individual patient and family needs. The role also supports bedside nurses with admissions, medication reviews, discharges, care coordination, and other assigned tasks; promotes evidence-based practice; advocates for patients and families; and facilitates learning for patients, families, and healthcare associates. This is an in-person opportunity at Nemours Children's Health's Jacksonville, Florida location. Current casual opportunities include day shifts (8 a.m.–8 p.m.), night shifts (8 p.m.–8 a.m.), or rotating shifts, including every third weekend.

Requirements

Associate of Science in Nursing (ASN) required.
Active Florida or multistate Registered Nurse (RN) license required.
At least one year of RN experience required.
BLS (American Heart Association) and PALS course completion required on hire.

Nice To Haves

Bachelor of Science in Nursing (BSN) preferred; a BSN or higher nursing degree must be from an accredited nursing program.
National certification in the area of clinical specialty preferred.
Experience in a pediatric specialty area preferred.

Responsibilities

Use policies, procedures, guidelines, and reliable methods to make clinical practice decisions.
Independently develop, implement, evaluate, and modify clinical practice using nursing knowledge, critical thinking, and nursing skills.
Collect and interpret complex patient data and clinical events to make decisions that address individual patient and family needs.
Use remote patient monitoring technology to identify clinical trends and respond promptly.
Monitor vital signs, weight, scoring tools, and other health indicators to support early detection of potential issues.
Communicate with patients and families in inpatient and home-based care settings using two-way video technology.
Conduct virtual visits to support bedside care and provide real-time assessments, guidance, and interventions.
Implement and monitor care delivery using clinical pathways, standards, guidelines, policies, and protocols.
Promote adherence to evidence-based practices to optimize patient outcomes.
Provide health teaching and counseling, develop individualized patient education plans, and provide discharge education.
Assist bedside nurses with admissions, patient education, medication reviews, discharges, care coordination, and other assigned tasks.
Collaborate with multidisciplinary teams, seek expert consultation, and delegate to other clinicians as appropriate.
Participate in team meetings and discussions about patient care and practice issues.
Provide professional nursing support remotely to patients and bedside Registered Nurses, enhancing bedside teams' ability to deliver high-quality care.
Assess, plan, implement, coordinate, monitor, and evaluate the patient's care plan from admission through discharge using virtual means.
Advocate for patients and families, promote ethical standards and patient rights, and serve as a resource for ethical and clinical concerns.
Identify educational needs and facilitate formal and informal learning for patients, families, and healthcare associates.
Serve as a mentor, coach, and preceptor based on unit needs and individual preferences.
