About The Position

The Care at Home Registered Nurse (RN) supports patients and families across the continuum of the medical home model. The RN completes in-home assessments, monitoring, care planning, and interventions for acute care patients using approved technology and clinical protocols in collaboration with the care team. The RN works directly with patients and families to provide exceptional care throughout the patient journey, enhance their experience, and provide education as needed.

The position is based at Cleveland Clinic Indian River Hospital in Vero Beach, Florida. The schedule is nights, 6:30 p.m. to 7:00 a.m., with weekend and holiday requirements.

Requirements

• Graduate from an accredited school of professional nursing (ADN or BSN program).
• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross.
• Advanced Cardiac Life Support (ACLS) through the American Heart Association or ALS certification through the American Red Cross, presented upon hire or within the first year of hire based on the department service level.
• One year of current acute care RN experience; licensed military RN experience will be considered.

Nice To Haves

• Bachelor of Science in Nursing (BSN).
• Technology experience, such as EPIC, radiology platforms, or Teams.
• Med/Surg or observation experience.

Responsibilities

• Provide direct nursing care.
• Implement, monitor, evaluate, update, and revise patient care plans.
• Monitor, record, document, and communicate patients’ conditions.
• Administer prescribed medications and treatments.
• Note and carry out physician and nursing orders.
• Assess and coordinate patients’ discharge needs.
• Educate patients and their families.
