About The Position

As a member of Corewell Health’s nursing team, the CNA provides patient care according to individual care plans, unit routines, and hospital policies. Under the direction of a Registered Nurse, the CNA contributes to patient care and supports patients and their families. The role provides direct day-to-day care to assigned residents or patients, documents care, maintains the patient environment and equipment, completes required CNA education, and participates in selected quality monitoring activities.

This is a full-time, day-shift position in Specialized - Home Care West, based at Continuing Care in Grand Rapids, Michigan. The schedule is 40 hours per week, 8 a.m. to 4:30 p.m., Monday through Friday, with variable weekends.

Requirements

• High school diploma or equivalent.
• Completion of a State of Michigan-approved CNA training course.
• Michigan Nursing Assistant certification through the Michigan Department of Licensing & Regulatory Affairs, Bureau of Community and Health Systems, required upon hire.
• Michigan driver’s license required upon hire.
• Basic Life Support (BLS) certification from the American Heart Association, excluding the stated Hospice exception, within 90 days; alternatively, BLS certification from the American Red Cross within 90 days.

Responsibilities

• Provide direct patient care under the direction of the Unit Manager or Licensed Nurse, in accordance with unit policies and procedures.
• Provide day-to-day care to assigned residents or patients, including dressing, bathing, transporting, and feeding.
• Document care provided as directed and in accordance with guidelines.
• Maintain patients’ environments, work areas, and equipment, and communicate equipment malfunctions to appropriate personnel.
• Complete and maintain the required number of CNA educational hours.
• Participate in selected unit quality monitoring activities under the direction of the Unit Manager or Nurse Leader.
