About The Position

As a member of Corewell Health’s nursing team, the CNA provides patient care according to individual care plans, unit routines, and hospital policies. Under the direction of a Registered Nurse, Unit Manager, or other licensed nurse, the CNA supports patients with daily care needs, documents care, maintains the patient environment and equipment, and participates in selected quality monitoring activities. The role is based in Specialized - Home Care West at the Continuing Care site in Grand Rapids. The schedule is 40 hours per week, 8 a.m. to 4:30 p.m., Monday through Friday, with variable weekends.

Requirements

• High school diploma or equivalent.
• Completion of a State of Michigan-approved CNA training course.
• Michigan Department of Licensing & Regulatory Affairs, Bureau of Community and Health Systems Nursing Assistant certification upon hire.
• State of Michigan driver’s license upon hire.
• Basic Life Support (BLS) certification through the American Heart Association within 90 days, or through the American Red Cross within 90 days.

Responsibilities

• Provide direct patient care under the direction of the Unit Manager or a licensed nurse, consistent with unit policies and procedures.
• Provide day-to-day care to assigned residents or patients, including dressing, bathing, transporting, and feeding.
• Document care provided according to guidelines.
• Maintain patients’ environments, work areas, and equipment, and report equipment malfunctions to appropriate personnel.
• Complete and maintain the required number of CNA educational hours.
• Participate in selected unit quality monitoring activities under the direction of the Unit Manager or Nurse Leader.
