About The Position

Advanced Care at Home (ACH) is a virtual care inpatient nursing unit based on the hospital-at-home model, providing high-acuity care to patients in their homes. Using telemedicine technology, ACH remotely monitors and provides care through videoconferencing for hospitalized patients at home, as an alternative or addition to established clinical settings across Mayo Clinic Enterprise. A multidisciplinary team of nurse practitioners, physician assistants, registered nurses, case managers, and pharmacists collaborates remotely from an ACH Command Center. The ACH RN performs patient assessments, medication verification, patient education, patient acquisition, and discharge activities. The nurse collaborates with the multidisciplinary care team to coordinate care needs and therapies in patients' homes. ACH RNs use the Medically Home platform and its supporting infrastructure to coordinate delivery of services in the home.
