About The Position

CareOne Concord is seeking an experienced RN Unit Manager for their Long Term Care, Short Term Rehab, Skilled Nursing, and Sub Acute Care units. This role is responsible for the day-to-day coordination and oversight of all aspects of an assigned clinical unit, ensuring high-quality resident care and regulatory compliance.

Requirements

  • Actively licensed RN in assigned state(s) of employment.
  • Previous supervisory experience.
  • Two years of LTC experience.
  • Graduate of an approved RN program.

  • Bachelor's degree preferred.
  • Acute Care experience preferred.

Responsibilities

  • Ensure required staffing levels are met for each shift and adjust as needed.
  • Update the posted schedule to reflect actual staffing.
  • Attend, participate, and lead the weekly resident 'At Risk' meeting to identify and address care concerns.
  • Attend and participate in morning clinical review meetings, PPS meetings, and monthly QA and A meetings.
  • Monitor the provision of resident care by licensed nurses and CNAs, ensuring documentation is complete.
  • Review the 24-hour resident change in condition report daily and ensure necessary follow-up.
  • Review resident medical records and documentation to ensure clinical programs are in place.
  • Identify staff educational needs and collaborate with the Facility Educator.
  • Participate on other committees as requested.
  • Ensure resident care and documentation comply with state and federal regulations.
  • Ensure compliance with HIPAA rules and regulations and report violations.
  • Make work assignments for assigned staff, including breaks and lunches.
  • Initiate disciplinary action as necessary in collaboration with DON/ADON.
  • Communicate with department managers/DON and participate in resolving grievances.
  • Ensure physician orders are followed.
  • Assist the DON/ADON with performance evaluations.
  • Greet new admissions and families and assist with the admission process.
  • Ensure all admission documentation is complete within 24-48 hours.
  • Monitor medication passes and treatment schedules.
  • Provide direct nursing care as necessary.
  • Report problem areas to the Director of Nursing and assist in developing corrective action.
  • Attend and participate in continuing education programs.
  • Ensure nursing staff follows policy and procedures, including dress code.
  • Ensure nursing staff participate in fire safety and disaster preparedness drills.
  • Ensure nursing personnel follow infection control procedures.
  • Ensure nursing equipment is operated safely.
  • Participate in the development of resident care plans.
  • Ensure resident care is provided in privacy.
  • Report and assist with investigation of allegations of resident abuse.
  • Assist Social Services with Advance Directive/Code Status documentation.
  • Participate in annual and complaint survey processes and assist with plans of correction.
