About The Position

SFAF’s Health Services for People Who Use Drugs (PWUD) is one of largest PWUD health engagement and safer supply access programs in the State of California. The Treatment Connections Navigator will build relationships with program participants at all sides, providing client centered connections to care. The role is grounded in a proactive counseling approcah that utilizes motivational interviewing to connect individuals to care. This position will work closely with Health Educators, Substance Use Treatment Coordinators and HCV Care Coordinators to engage and link people to internal and external substance use and medical care services. Linkage for participants will include connecting, escorting, and following up to ensure that they are receiving effective and culturally competent care.

Requirements

  • Minimum 3 years experience working with people who use drugs, people experiencing homeless, and other highly marginalized groups required.
  • Ability to speak candidly and knowledgeably about drug use, substance use treatment options, and sexual risk.
  • Ability to implement de-escalation techniques, follow safely protocols, and use sound judgment in making independent decisions.
  • Knowledge of substance use and health concerns of people who use drugs (PWUD) and the services available to them in San Francisco.
  • Ability to provide culturally sensitive and stigma free customer service to program participants.

Nice To Haves

  • Bi-lingual in Spanish and English is highly regarded.

Responsibilities

  • Provide on site treatment connections including referral and linkage to substance use disorder treatment, medical care, HIV, HCV/STI Testing, vaccinations, and other health and social services.
  • Provide active counseling, motivational interviewing style substance use treatment readiness coaching, risk reduction counseling, and substance use treatment adherence counseling.
  • Create a warm and welcoming environment for people who use drugs, with an emphasis on excellent customer service and compassionate care
  • Provide on-site health education and substance use treatment awareness counseling at both mobile sites and 6th St Center
  • Provide health coaching focused on overdose prevention, HIV and HCV prevention, and other health improvement activities
  • Document and enter all site data and referral data according to program protocols
