The Treatment Resource Navigator will focus on providing full-time street outreach services to individuals and families experiencing homelessness in Milwaukee County that need medically assisted treatment and harm reduction services. This position will also work to secure indoor placements for consumers experiencing homelessness to include street outreach services, case management, and referring to local supportive housing programs.

Requirements

  • Current resident of the State of Wisconsin.
  • Current Wisconsin Driver's License maintained throughout employment.
  • Must possess a bachelor’s degree or higher in Social Work, Human Services, or closely related field.
  • At least one (1) year of experience working with the homelessness population, supportive housing, and/or human services, including substance abuse and harm reduction.
  • Intermediate knowledge of all computer and software applications such as Microsoft Word and Microsoft Excel.
  • Must be able to travel extensively, up to 70% of work time, within the community.

Responsibilities

  • Locate and engage with individuals and families experiencing unsheltered homelessness.
  • Collaborate with outreach teams to develop immediate housing plans and refer to housing programs.
  • Develop and strengthen relationships to connect participants to harm reduction and medically assisted treatment services.
  • Acquire benefits.
  • Establish community partnerships with agencies to assist individuals/families connect to harm reduction services.
  • Work with existing and potential landlords to find additional units for the program.
  • Perform other duties as assigned.
