About The Position

RN Navigators at Coordinated Treatment Center collaborate closely with multidisciplinary teams across a diverse range of patient populations and diagnoses to coordinate comprehensive, patient-centered care. This role integrates health care for patients/clients across the continuum of care. The collaboration with the healthcare team and other professionals ensures timely sequencing of care, enhancing patient flow and interdisciplinary communication to promote early intervention. The role supports patients/clients and their family members by removing barriers to care. It seeks to evaluate outcomes based on an integration among established clinical, financial, and utilization data. The position functions in structured and unstructured healthcare settings, which may not have established policies and procedures, utilizing independent nursing judgment. Knowledge of and utilization of appropriate age-related patient care protocols for pediatric, adolescent, adult, and geriatric patients is essential. The RN Navigator may serve as a key link between providers, clinics, insurance concerns, communication, inpatient floors, and outside facilities, establishing and maintaining good interpersonal relationships. The role requires the ability to work with the growth and development needs of client populations in the clinical area, strong communication skills, and the ability to coordinate, facilitate, and negotiate with others. Monitoring, evaluating, and trending patient responses using structured care methodologies, working closely with ancillary and support services, and the ability to solve problems and identify/implement process improvements are key. The role requires self-motivation with little direction and the ability to facilitate groups, multidisciplinary teams, and implement best practices. The RN Navigator functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in the state of practice and licensure, adhering to the American Nurses Association's Scope and Standards of Practice and Code of Ethics for Nurses.

Requirements

  • Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • Obtains and subsequently maintains required department specific competencies and certifications.

Nice To Haves

  • Bachelor's degree in Nursing strongly preferred in Bemidji and Rapid City.
  • Required in Fargo and Sioux Falls (Sanford USD Medical Center and Clinics).
  • Required in Bismarck or enrolled and starting a Bachelor’s degree program within one year of hire with a completion date within three years upon date of hire into position.
  • Required in Fargo Network and Sioux Falls Network or employees hired after January, 2025, who do not possess a Bachelor’s degree in the required field, leadership may consider an educational plan with proven continuous action toward achieving a Bachelor’s degree in Nursing within five years of hire into position.
  • Approval from the Nursing Executive for the market is required in order to waive the bachelor degree requirement (at time of hire).
  • Additionally, for Rural Health Network facilities, for current employees, leadership may have considered acceptable qualifications and work experience equivalency based on facility size, rural market, and business need, prior to new Sanford education requirements.
  • Minimum of two years’ clinical experience preferred.

Responsibilities

  • rooming patients, obtaining vitals, weights, and other clinical measures
  • supporting team members to ensure patients are seen by the appropriate disciplines
  • monitoring patient progress to confirm orders and education are completed
  • addressing ongoing patient needs through phone, MyChart, and other follow-up communication methods

  • May serve as a key link between providers, clinics, insurance concerns, communication, inpatient floors and outside facilities.
  • Establishes and maintains good interpersonal relationships with those with whom has contact.
  • Able to work with growth and development needs of client populations in clinical area.
  • Strong communication skills are required.
