About The Position

This role involves managing a caseload of clients, ensuring their progress through treatment programs, and facilitating their reintegration into the community. The Treatment Case Manager is responsible for maintaining accurate and confidential client records, collaborating with a treatment team, and connecting clients with necessary community resources. This position requires crisis intervention, client advocacy, and adherence to program policies and documentation requirements. The role also involves coordinating with medical providers, assisting clients with practical needs like housing and insurance, and ensuring compliance with treatment rules and expectations.

Requirements

  • High School diploma or equivalent required.
  • Must complete 40 hours of training in the first 60 days of hire to become a Qualified Behavior Health Specialist under the Medicaid guidelines.
  • Must be eligible and willing to obtain a CDCA within three months of working in the facility.
  • Candidates must be eligible to work in the U.S. without requiring sponsorship.

Nice To Haves

  • Associate’s Degree in Behavioral Health or related field and LCDC II or above.
  • Strong organizational skills and detail-oriented.
  • Excellent verbal and written communication skills.
  • Reliable transportation, a valid Ohio driver’s license, and a driving record that does not preclude the employee from being covered by Agency liability insurance.
  • Ability to work effectively with Agency employees, outside contacts, and a diverse client population.

Responsibilities

  • Prepare and maintain confidential, accurate, organized, and updated case files and EHR for each assigned client.
  • Ensure timeliness of documentation as per departmental policies/protocols.
  • Comply with documentation requirements for assessments, treatment plans, treatment procedures, and reports.
  • Identify and respond to crises, using negative events to enhance treatment efforts.
  • Initiate contact with Crisis Intervention Counselor for support/intervention when needed.
  • Identify and respond to client barriers to program completion (e.g., age, education, medical, mental health, transportation).
  • Monitor client behaviors and symptoms that may impact level of care recommendations and notify Treatment Counselor of potential reassessment needs.
  • Facilitate assigned Treatment plus group(s).
  • Maintain communication with Agency’s contract doctor, consulting psychiatrist, and/or community medical providers regarding medication.
  • Alert identified staff of any changes in client adjustment suggesting decompensation.
  • Initiate communication and/or meetings with client’s loved ones to promote successful community integration.
  • Assist clients in becoming positive, productive, prosocial community members.
  • Act as an advocate for clients and hold them accountable to treatment rules and expectations.
  • Collaborate with treatment team members (clinicians, Cognitive Skills Specialist(s), Recovery Coaches) to ensure client needs are met.
  • Manage caseload effectively, organizing work demands and meeting deadlines.
  • Meet with clients regularly in accordance with program procedures.
  • Ensure client attendance in primary treatment groups, plus groups, and enhanced programming, assisting with barrier identification and problem-solving.
  • Prepare and maintain accurate records, documents, and outside correspondence.
  • Participate in team meetings, discussing client issues and case reviews.
  • Maintain familiarity with and utilize community resources, completing release of information forms for referrals.
  • Transport clients to appointments safely and in accordance with agency policies, completing interventions during transportation for billable activities.
  • Effectively manage time on and off Oriana House, Inc., premises while on duty.
  • Coordinate necessary insurance-related appointments to ensure clients have applied for or have medical insurance.
  • Make referrals based on client's ongoing needs as reflected in the Individual Treatment Plan.
  • Assist clients with making suitable housing arrangements.
  • Ensure warm hand-offs in the community by transporting/linking clients with resources post-release.
  • Recognize the need for case consultation with supervisors by participating in case reviews and seeking assistance when needed.
  • Maintain professional training to complete licensure examination and receive license within the expected timeframe.
  • Maintain required licensure and/or certification for job assignment.
  • Recognize and effectively address and redirect clients’ anti-social behaviors.
  • Collect client UDS and alcohol screens as needed and assigned.
  • Continue client engagement post-release from facility for active clients to ensure continued success in accessing resources (FAA Specific).
