About The Position

As the Case Manager, you will be responsible for the planning and treatment of individual children, a family, or assigned groups by performing a variety of regular tasks. This role involves developing care plans, coordinating with clients and families, documenting progress, and providing a supportive environment for youth. You will also serve as a liaison with referring agencies and oversee the general health and well-being of the child.

Requirements

Bachelor's Degree in social work or a degree in a human services related field plus two years of experience in child welfare or a Master's Degree in Human Services is required
Prior experience in working with Foster and Adoptive families is required
Able to be self driven/reliant and work independently as well as part of a team
Strong organizational skills
Ability to show empathy/compassion to build trust based relationships
Effective verbal and written communication skills

Responsibilities

Developing a care plan for each child containing goals and objectives with projected dates of accomplishment
Involving the clients, referral agency and family in planning whenever possible
Developing a specific plan relating to the involvement of the child’s parents
Participating in the documentation of case assessment
Documenting the implementation of the care plan
Providing for scheduled in-person conferences with each resident
Providing a supportive atmosphere for the youth with leadership and guidance to the client
Coordinating internal and external activities of the youth
Serving as a liaison with the referring agency
Providing a mechanism for dealing with day-to-day program operations
Oversees and maintains general health and well-being of the child
Overseeing the living activities of the child
Monitoring and recording behaviors on a daily basis
