About The Position

The Travel Registered Nurse - Hospice provides comprehensive hospice nursing care to assigned patients by coordinating and delivering assessments, care planning, interventions, and evaluations in alignment with the interdisciplinary plan of care. The role involves extensive nationwide travel to support hospice agencies experiencing staffing needs, while maintaining clinical excellence, regulatory compliance, and compassionate patient- and family-centered care.

Requirements

Current, unencumbered Registered Nurse (RN) license in the state(s) of assignment; a multi-state Nurse Licensure Compact (NLC) license is required where applicable.
Current CPR certification.
3+ years of experience as a Registered Nurse in hospice or home health care.
Valid driver’s license, reliable transportation, and required liability insurance.

Responsibilities

Travel extensively throughout the company on a regular basis to provide patient care to agencies in need of permanent staff.
Perform initial nursing assessments for new admissions.
Complete, coordinate, and revise the total plan of care, maintaining continuity of patient care through collaboration with appropriate staff.
Communicate patient progress regularly to the clinical manager, physician, and care team.
Develop, prepare, and maintain accurate and timely individualized patient care progress records in accordance with care center policies.
Record changes and outcomes related to pain and symptom management as appropriate.
Educate families about patient condition, needs, and care.
Refer patients to other hospice care disciplines as needed, or document the rationale for not doing so.
Perform assigned on-call responsibilities and provide on-call services to patients and families.
Perform other duties as assigned.
