About The Position

This is a travel assignment for a Registered Nurse specializing in Hospice care in Grand Junction, Colorado. The assignment begins on 09/14/2026 and lasts for 12 weeks, with a guaranteed 40 hours per week. The role involves working a 5x8-day shift schedule and providing compassionate, patient-centered hospice care within an inpatient setting.

Requirements

Ability to work in an inpatient hospice setting with occasional on-call and weekend rotations.

Responsibilities

Deliver direct patient care with a focus on symptom management, comfort, and quality of life for hospice patients.
Lead the interdisciplinary care team for assigned patients, coordinating medical, psychosocial, and spiritual support.
Perform comprehensive assessments, monitor clinical changes, and document care plans and outcomes.
Educate patients and families about goals of care, treatment options, and ongoing symptom management.
Collaborate with the care team to implement and adjust plans across care settings, including private homes, nursing facilities, group homes, and assisted living facilities.
Ensure timely documentation, regulatory compliance, and adherence to hospice guidelines and standards.
