About The Position
This is a travel Registered Nurse position specializing in hospice care, located in Sonora, California. The assignment is for a 13-week duration within a short-term acute care hospital environment that includes a hospice program. The role involves providing comprehensive care to patients and their families, managing caseloads, and collaborating with an interdisciplinary team. The position offers guaranteed hours and mileage reimbursement.
Requirements
- Registered Nurse (RN) license in the state of practice (California) – Required
- Current CPR or Basic Life Support (BLS) certification from an approved vendor – Required
- Valid driver’s license, at least 21 years old, current automobile insurance – Required
- At least two years of recent nursing experience in the field; hospice experience required for travel assignment
- Ability to work in a hospital-based hospice setting and to coordinate care across disciplines

Responsibilities
- Conduct initial and ongoing assessments of patients’ physical, emotional, psychosocial, and spiritual needs; develop and update individualized care plans with the interdisciplinary team
- Deliver skilled nursing care, including medication management, pain and symptom control, wound care, and patient/family education on disease process and hospice philosophy
- Provide emotional support and crisis intervention to patients and families as needed
- Serve as the primary case manager for an assigned caseload (typically 12–18 patients), ensuring continuity of care
- Communicate regularly with physicians, social workers, chaplains, hospice aides, and other team members; participate in interdisciplinary team meetings
- Document accurately in the electronic medical record (EMR) in compliance with regulatory requirements; ensure compliance with Medicare/Medicaid and state/federal hospice regulations
- Participate in the on-call rotation and respond promptly to after-hours patient needs
