About The Position

The Trauma Outreach Coordinator provides a leadership role in trauma prevention outreach with an internal and external focus utilizing the Trauma Registry to identify and address institutional and community needs. The role involves developing, implementing, and monitoring a comprehensive injury prevention program suited to all ages to meet the needs of patients, families, staff, and the community. This position also collaborates with various organizations in the development and implementation of trauma outreach, and is responsible for the overall coordination, organization, implementation, evaluation, and tracking of the Trauma Injury Prevention and Outreach Program Activities. Additionally, the coordinator serves on and/or facilitates committees, councils, and task forces as assigned, and provides a coordinated care experience for patients/families throughout the continuum of care to ensure optimal outcomes. Program evaluations for efficacy, efficiency, quality, and appropriateness are also a key function, along with assisting in performance improvement activities.

Requirements

  • Bachelors degree required in related field.

Nice To Haves

  • Knowledge of Injury Prevention, epidemiology and grant application preferred.

Responsibilities

  • Provides a coordinated care experience for patients/families throughout the continuum of care to ensure that optimal physical, psychological and functional outcomes are met, to manage.
  • Evaluates programs and presentations to efficacy, efficiency, quality of presentation and appropriateness of the information including feedback from requesting agencies.
  • Assists in performance improvement activities as required.
