About The Position

As a Transition Specialist, your role is to set patients up for success to transition out of the hospital into their next stage of care. You will be the "sigh of relief" for patients as they transition out of the hospital. Most of your day is spent interacting face-to-face with patients. You work collaboratively as a critical part of the clinical care team. PEA offers unique career path options that allow Transition Specialists to grow and develop.

Requirements

• High School Education or equivalent required
• 6 months pharmacy technician (retail preferred) OR 1-year medical assistant / patient care technician / patient care coordinator / clinic front desk associate / nursing assistant / home health aide / other healthcare or medical experience required
• Other related healthcare, hospital, or medical experience will be considered
• Ability to report to work onsite as scheduled; ability to be on feet – standing, walking, and/or climbing stairs 8-10 hours daily
• Ability to receive required vaccinations (as required by PEA or hospital) unless approved for exemption

Responsibilities

• Schedule patient’s follow-up appointments
• Schedule patient’s transportation as needed
• Deliver discharge prescription medications to patient’s bedside
• Assist patients with medication affordability
• Coordinate home meal delivery service for patients
• Connect patients to community resources as needed
