The role serves adults with severe and persistent mental illness who may be at risk of hospitalization, suicide, violence, or significant functional impairment. As part of a multidisciplinary team, the therapist helps clients transition from psychiatric inpatient care to community living through therapy, crisis intervention, care coordination, and ongoing support intended to promote stability, recovery, and independence. Services are delivered in office-based and community settings, with collaboration among clinicians, case managers, peer specialists, recreation therapists, and other professionals. The position is scheduled Monday through Friday, 8:00 a.m. to 5:00 p.m., with a hybrid arrangement of two days in the office and three days working remotely. The schedule may be adjusted to address program, training, client, and community needs. The position is listed as 1.0 FTE (40 hours per week) and benefited.
