This full-time position involves providing patient/family-centered care as a Telephone Nurse Triage for the Neurology department. The role requires assessing nursing needs of acute and chronically ill patients through telecommunication technologies such as telephone, fax, email, internet, or patient portal. The nurse will act as a partner and adviser, assisting patients and families in optimally managing their healthcare, respecting their culture, values, individual needs, health goals, and treatment preferences. Key responsibilities include applying critical reasoning and clinical judgment to expedite appropriate care, ensuring proper scheduling and follow-up on test results, using active listening skills and approved protocols to explain results and medications, and adhering to the American Nurses Association Code of Ethics. The role also involves participating in professional development, shared governance, and promoting patient safety and quality nursing care through appropriate interventions, health education, patient advocacy, care coordination, and outcome evaluation. Adaptability in communication style to meet diverse patient needs and exceptional assessment, critical thinking, and customer service skills are essential.
