This full-time position involves providing patient/family-centered care as a partner and advisor, assisting patients and families to optimally manage their healthcare. The role requires applying critical reasoning and clinical judgment to expedite appropriate care and treatment, often assessing patients through telecommunication technologies such as telephone, fax, email, internet, or patient portal technology. Responsibilities include ensuring proper scheduling and follow-up on test results for providers, using active listening skills, approved protocols, and evidence-based practice to explain test results and medications. The nurse will adhere to the American Nurses Association Code of Ethics, participate in professional development, and focus on patient safety and quality of care. This includes identifying and clarifying patient needs, conducting health education, promoting patient advocacy, coordinating services, assisting patients in navigating the healthcare system, and evaluating outcomes. Maintaining knowledge of care teams for coordination, self-management, population management, and communication skills is essential. The role requires adapting communication style to diverse patient backgrounds and demonstrating exceptional assessment, critical thinking, and customer service skills.
