About The Position

Children's Hospital of Philadelphia (CHOP) is seeking a part-time After-Hours Triage nurse. The shift will be dependent upon the operational need and includes weekends and holidays. This role requires a nurse who practices at the level of a competent to proficient practitioner of nursing, demonstrating competence in Telehealth nursing and clinical leadership skills. The nurse will contribute to standards of care and unit goals, serve as a role model and preceptor, and demonstrate competence in caring for patients with a wide variety of health issues. The nurse will also assist the Manager with administrative issues affecting clinical practices and comply with administrative policies and clinical protocols. Administratively, the nurse is responsible to the Manager and Ambulatory Nursing Director for nursing practice issues.

Requirements

Technical Diploma Nursing - Required
At least five (5) years registered nursing experience - Required
Registered Nurse (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Required
Registered Nurse (New Jersey) - New Jersey State Licensing Board - upon hire - Required
Computer Competency (Required proficiency)
Basic use of a computer (mouse, keyboard, printer, USB ports). (Required proficiency)
Basic use of Microsoft Office (Word processing, spreadsheets, presentations and Skype). (Required proficiency)
Web browsing, intranet search, document access. (Required proficiency)
Use of shared network file space. (Required proficiency)
Navigation and use of clinical communication systems. (Required proficiency)
Determines the nature and extent of clinical information needed and uses the appropriate technology to access it. (Required proficiency)
Accesses needed information effectively and efficiently. (Required proficiency)

Nice To Haves

Bachelor's Degree Nursing - Preferred
Pediatric Experience - Preferred
Preferred telephone triage and/or Barton Schmitt triage protocol experience.

Responsibilities

Follows the Nursing Process to deliver safe telephone triage care: assessment, planning, implementation, and evaluation.
Collects and synthesizes complex assessment data and uses critical thinking skills to determine the most appropriate protocol and disposition.
Develops a plan of care.
Communicates and collaborates with families/caregivers on the plan of care, ensuring understanding and compliance.
Ensures documentation reflects all aspects of telephone encounters.
Collaborates with the PCP and other healthcare providers as necessary.

Applies education and training from departmental in-services and external sources to enhance Nursing practice.
Assists in developing and utilizes patient education materials relevant to the patient population.
Participates in the development/presentation of staff education programs geared to meet unit and select departmental needs.
Demonstrates health care informatics competency to manage patient care.
Demonstrates information literacy, including the effective use of the intranet and internet to find electronic resources for patient care, nursing practice, and evidence-based decision-making.
Demonstrates information management skills, including the effective use of the electronic health record and clinical information systems, the ability to collect clinical data using quality tools, and the ability to discuss, evaluate, and analyze outcome data presented in charts and tables.
Participates in promoting quality, efficient, and cost-effective care.
Monitors own usage of supplies and equipment/identifies opportunities for improvement and makes recommendations.
