About The Position

Telos Health Systems is hiring Nurse Practitioners, Physician Assistants, and Psychiatric-Mental Health Nurse Practitioners to provide telemedicine care to geriatric and post-acute patients at skilled nursing facilities throughout North Carolina. The role combines acute and routine medical care with behavioral health support, including assessment of common psychiatric, behavioral, and cognitive concerns. Providers work with bedside nursing teams to support treat-in-place care and determine when escalation or transfer is appropriate. The position serves patients with varying degrees of medical, cognitive, and psychiatric complexity. Providers respond to urgent calls, conduct scheduled medical and behavioral health follow-ups, evaluate common SNF concerns, and provide coverage during planned or unexpected provider absences. Work is conducted through telemedicine workflows, supported by established protocols, onboarding, mentorship, clinical consultation, and operational and scheduling support. Providers are expected to exercise independent clinical judgment and document encounters in the EHR. This is a W2, part-time, fully remote position with day and night shift opportunities. The minimum commitment is 24 hours per week across two 12-hour shifts. Scheduling is collaborative but requires flexibility to help meet changing coverage needs, including weekend shifts, night shifts, and one major or minor holiday annually. Additional shifts may be available as the service line grows.
