About The Position

Telos Health Systems is seeking Nurse Practitioners, Physician Assistants, and Psychiatric-Mental Health Nurse Practitioners to provide telemedicine care to geriatric and post-acute patients in skilled nursing facilities throughout North Carolina. The role combines acute and routine medical care with behavioral health support, including evaluation of common psychiatric, behavioral, and cognitive concerns. Providers work with bedside nursing teams to support treat-in-place care and determine when escalation or transfer is appropriate. The position involves responding to urgent and acute calls, completing scheduled medical and behavioral health follow-up visits, assessing common medical and behavioral health concerns, developing treatment recommendations, and documenting encounters in the EHR. Providers work independently within their scope of practice, using established protocols and telemedicine workflows, and collaborate with facility staff, other clinicians, and telehealth leadership. The patient population has varying degrees of medical, cognitive, and psychiatric complexity. This is a W2, part-time, fully remote position requiring at least 24 hours per week across two 12-hour shifts. Day and night shifts are available. Scheduling is collaborative, but providers must be willing to help meet changing coverage needs, including weekend and holiday shifts. Telos provides paid onboarding, mentorship, clinical consultation, operational and scheduling support, and home-office technology equipment.
