About The Position

Telos Health Systems is hiring Nurse Practitioners, Physician Assistants, and Psychiatric-Mental Health Nurse Practitioners to provide telemedicine care to geriatric and post-acute patients in skilled nursing facilities throughout North Carolina. The role combines acute and routine medical care with behavioral health support, including assessment of common psychiatric, behavioral, and cognitive concerns. Providers work with substantial clinical autonomy while collaborating with bedside nursing teams and following established protocols and escalation workflows to support safe treat-in-place care. The patient population has varying degrees of medical, cognitive, and psychiatric complexity. Providers respond to urgent and acute calls, conduct scheduled medical and behavioral health follow-ups, evaluate and manage common concerns within their scope of practice, recommend treatment plans, and determine when emergency transfer or a higher level of care is appropriate. They also provide temporary coverage during provider absences and coverage gaps across Telos facilities. The role includes accurate EHR documentation, onboarding and training, and occasional team meetings. Structured protocols, mentorship, clinical consultation, operational support, and comprehensive paid onboarding are provided. This is a W2, part-time, fully remote position requiring at least 24 hours per week, consisting of two 12-hour shifts. Shifts are scheduled days or nights, with flexibility needed to meet evolving coverage needs. Providers are expected to work one to two weekend shifts per month, one to three night shifts per month, and one major or minor holiday annually. Additional shifts may be available as the service line grows.
