Registered Nurse (RN) Post Acute Care, Skilled Nursing - PRN
About The Position
Wythe County Community Hospital is seeking a Registered Nurse (RN) to provide post-acute care in its skilled nursing unit. The unit has 8 beds, supportive nurse-to-patient ratios, and a manager who prioritizes patient safety and positive outcomes. The role offers schedule flexibility, learning and professional development opportunities, and a clinical ladder program. The RN assesses patients, identifies their needs, initiates appropriate nursing interventions, and provides care suited to each patient's condition and age. The nurse documents medical necessity in the medical record, completes and revises each patient's Interdisciplinary Care Plan, and performs timely and accurate quality improvement assessments.
Requirements
- Active Virginia single-state RN license or compact license. If relocating, licensure must be updated to the new Primary State of Residence before employment begins.
- Associate's degree required.
- Basic Life Support (BLS) certification required within 30 days of hire.

Responsibilities
- Accurately perform patient assessments and identify patient needs.
- Identify and initiate appropriate nursing interventions.
- Provide care appropriate to the patient's condition and age.
- Complete timely and appropriate documentation relating to medical necessity in the medical record.
- Complete and revise the Interdisciplinary Care Plan for each patient.
- Perform timely and accurate quality improvement assessments.
