The Cleveland Clinic Critical Care Advanced Practice Provider (APP) Fellowship is an intensive, 12-month post-graduate program preparing Acute Care Nurse Practitioners and Physician Assistants to provide comprehensive critical care services to critically ill Medical ICU patients. Fellows develop critical-thinking, diagnostic, and clinical skills for managing complex cases and delivering safe, efficient, evidence-based care within a multidisciplinary, team-based model. The program has two cohorts per year. Training begins with a boot camp and includes weekly protected didactic education, case-based learning, lectures, simulations, and nine months of precepted rotations in intensive care units and other services, followed by three months of internship. Clinical schedules vary by rotation and include days, evenings, nights, weekends, and holidays. Fellows commit approximately five days and 50 hours per week to clinical and didactic training. Training includes procedural simulation, procedural-service rotations, and dedicated airway-management training in the operating room. Department APPs and physician experts supervise the fellows. Each fellow also receives professional-development instruction and one-on-one mentorship to identify and complete a quality or research project intended for submission as an abstract or peer-reviewed article. The fellowship is concentrated at Cleveland Clinic Main Campus, Fairview Hospital, and Hillcrest Hospital in Northeast Ohio. Main Campus rotations include the Medical ICU, Medical Intensive Liver Unit, Procedure Service, and Consult Service; Fairview and Hillcrest rotations include their Medical ICUs and Coronary Care Units. The Department of Critical Care Medicine serves 12 adult ICUs across nine Cleveland Clinic hospitals in Northeast Ohio, including the 64-bed Medical ICU at Main Campus. The fellow learns the Advanced Practice Provider role under the supervision of an APRN/PA preceptor and/or physician preceptor for no longer than 12 months. Duties include patient assessment, diagnostic testing and interpretation within privileges, treatment planning under physician direction, patient education, documentation, referrals, care coordination, research support, quality monitoring, and discharge planning. The role requires critical thinking, decisive judgment, and the ability to work in a stressful environment. The fellow is hired into a full-time, salaried position for a term not exceeding one year and is eligible for full-time benefits described in Cleveland Clinic’s Total Rewards Brochure. Salary is paid semi-monthly. After successful completion of the fellowship, the fellow may apply for open positions within Cleveland Clinic; employment after the fellowship is not guaranteed. The listed cohort start dates are April 5, 2027, and October 4, 2026.
