Telehealth Patient Specialist – Ambulatory Care- Boise
About The Position
St. Luke’s is hiring an experienced Telehealth Patient Specialist to support its Ambulatory Care and Women’s Services programs. The role connects patients with their care teams through remote patient monitoring, care coordination, and telehealth services. The specialist helps improve access to care and patient engagement, and supports patients in managing their health from home while partnering with multidisciplinary teams across service lines. The position is full-time, with three 12-hour shifts per week from 7:00 AM to 7:30 PM. Rotating weekends and holidays are required. The pay range is $18.37–$20.27 per hour, depending on experience.
Requirements
- High school diploma or equivalent.
- At least 6 months of experience.
Nice To Haves
- Experience as a Certified Nursing Assistant (CNA), Unit Clerk, Medical Assistant, Patient Access Specialist, or in a similar healthcare support role.
- Familiarity with medical terminology.
- Experience in a healthcare, ambulatory, telehealth, call center, women’s health, or patient-facing environment.
- Strong customer service, communication, and computer skills.
Responsibilities
- Onboard patients into remote patient monitoring, chronic care management, and Women’s Services programs as applicable.
- Monitor patient participation and assist with adherence to established care plans.
- Help patients set up and use remote monitoring equipment and technology.
- Coordinate scheduling, follow-up appointments, and patient outreach.
- Communicate with patients, providers, nurses, and interdisciplinary care teams.
- Use medical terminology and healthcare knowledge to support patient interactions and documentation.
- Support initiatives to improve patient outcomes and experience and reduce avoidable hospital utilization.
- Provide compassionate support across ambulatory and women’s health care settings.
- Perform other duties and responsibilities as assigned.
