Team Leader - Aged Care

About The Position

The Team Leader plays an integral role in providing high-quality care to residents. The position supports the Clinical Care Coordinator and Registered Nurse in supervising and coordinating care across the home, while ensuring each resident’s individual needs, wishes and goals are met in an environment where they feel supported, valued, and treated with dignity and respect. The role is open to Endorsed Enrolled Nurses or Personal Care Workers with appropriate qualifications.

Requirements

Diploma of Enrolled Nursing with AHPRA registration to administer medication; or Certificate IV in Ageing Support, including HLTHPS007 Administer & Monitor Medicines; or Certificate III in Individual Support with willingness to undertake training to obtain required qualifications.
High level of commitment, integrity, honesty and trustworthiness.
Effective listening skills and ability to communicate orally and in writing at all levels.
Ability to understand and use information technology and programs, and to learn new skills as required.
Current National Police Check.
Working knowledge of workplace health and safety (WH&S) and infection control.
Desire to join a team committed to caring for others in a mission-based organisation.
Evidence of immunisation as required for the role.
Successful completion of applicable pre-employment checks, which may include reference checks, work rights, criminal history, NDIS/WWCC/WWVP checks and a health evaluation.

Responsibilities

Provide social and emotional support to residents, ensuring care is holistic and person-centred.
Support coordination of residents’ care needs across the home, including medication management, nutrition and hydration, and pressure care management.
Lead and supervise care during the shift within the designated wing or area.
Perform medicine-related tasks within the scope of qualification and in accordance with applicable state or territory legislation, regulations, and Calvary policies and procedures for delegation and supervision.
