About The Position

Moments Hospice is hiring a Team Director (RN Clinical Manager) to lead an interdisciplinary hospice care team. The role guides and mentors clinicians, coordinates care with patients, families, physicians, and other providers, and ensures high-quality, compliant, holistic care that respects each patient's needs and beliefs. The position is full-time and field-based, with a company vehicle available through the fleet car program for business and personal use at minimal cost, subject to eligibility.

Requirements

• Active Registered Nurse (RN) license in the state where the employee works.
• Proficiency with electronic medical records (EMR).
• Strong understanding of the hospice philosophy and end-of-life care.
• Strong organizational, communication, and critical-thinking skills.
• A compassionate, patient- and family-centered approach.
• Valid driver's license and reliable vehicle with current insurance.

Nice To Haves

• Leadership or management experience is a major plus.

Responsibilities

• Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, bereavement coordinators, and volunteers.
• Provide clinicians with real-time guidance, mentoring, and clinical problem-solving support.
• Lead interdisciplinary group (IDG) meetings and ensure plans of care are accurate, current, and well documented.
• Conduct clinical record reviews, case reviews, and home visits as needed to ensure quality care.
• Ensure compliance with Medicare hospice regulations and maintain survey readiness.
• Manage staff scheduling while fostering a positive, collaborative team culture.
• Develop and update patient care policies and emergency preparedness procedures.
• Serve as the central point of communication between patients, families, physicians, other providers, and the care team.
• Maximize resource utilization to support efficient, high-quality care.
