About The Position

AccentCare is hiring a full-time Hospice Registered Nurse Team Director to supervise direct patient and family care team members in collaboration with program site leadership. The role combines clinical team oversight with administrative responsibilities, including scheduling and payroll. The director ensures required assessments and reassessments are completed by qualified team members within required time frames, with documentation submitted or synchronized as needed.

The director oversees and supports the Interdisciplinary Group (IDG) in developing, implementing, and reviewing each patient’s and family’s interdisciplinary plan of care. Working with site leadership, the director monitors, manages, and mentors staff on patient care activities, productivity, and documentation; supports staff education; reinforces case-management skills and practices; facilitates IDG meetings; and provides ongoing IDG support. The role also participates in or leads quality-improvement activities and supports internal and external communications. A licensed driver able to travel to all business locations is required, and the employee must meet the regulations and requirements of the state or states where the program provides services.

Requirements

• At least three years of experience in hospice, home health, general medical-surgical nursing, oncology, or patient-population-specific nursing.
• At least one year of supervisory experience.

Nice To Haves

• Associate degree preferred.
• Certified Hospice Nurse (CHPN) preferred.
• Valid CPR certification while employed in a clinical role is preferred.

Responsibilities

• Supervise direct patient and family care team members in collaboration with program site leadership.
• Manage scheduling, payroll, and other administrative duties.
• Ensure qualified team members conduct initial assessments and required reassessments within required time frames, including documentation submissions and/or synchronization.
• Oversee and support the Interdisciplinary Group (IDG) in developing, implementing, and reviewing each patient’s and family’s interdisciplinary plan of care.
• Collaborate with program site leadership to monitor, manage, and mentor staff in patient care activities, productivity, and documentation in accordance with related policies and protocols.
• Participate in and promote Back to Bedside efforts to reinforce case-management skills, knowledge, and attitudes across disciplines in accordance with protocol.
• Collaborate with site leadership to support staff education efforts.
• Facilitate IDG meetings and provide ongoing IDG support, including triaging incoming calls, assisting callers, and performing quality calls.
• Participate in or lead quality-improvement activities, including data collection and aggregation as able.
• Reinforce and support internal and external communications.
