The Supervisor, RN is responsible for making daily compliance rounds on units to observe, examine, or interview residents, verify information from the ongoing shift, monitor regulatory compliance, and determine staff assignments. This role involves providing orientation to unit routines for new staff, describing care plan goals to direct care staff to ensure consistent implementation, and seeking input from nurse aides regarding residents' conditions and care approaches. The Supervisor monitors the delivery of care and services to ensure resident needs are met and the quality of work by direct care staff is acceptable. They administer medications, treatments, and provide direct care to residents according to physician orders and facility policies. Additionally, this role involves performing physical assessments, developing and implementing resident care plans with measurable objectives, coordinating care with all disciplines, and consulting with interdisciplinary team members. The Supervisor trains and develops staff, conducts skills observations, identifies learning needs, and reports information to DON Unit Managers for staff performance evaluations. They complete required documentation of care, services, special circumstances, and accident/incident reports. A written 24-hour facility report is also completed. The Supervisor gives and receives verbal shift reports, noting significant changes in resident condition, admissions, transfers, discharges, use of restraints, medication errors, loss of property, or resident/family dissatisfaction. They are responsible for immediately informing the resident, consulting with the physician, and notifying the designated family member or legal representative in cases of accidents with potential for physician intervention, significant changes in resident status, or decisions to transfer/discharge. Prompt notification to the resident, family, or legal representative is also required for changes in room or roommate. The Supervisor reports relevant information to the DON and Unit Manager, including interdisciplinary team functioning, regulatory compliance, resident risk factors, behavioral changes, dissatisfaction, treatment refusals, unexplained injuries, staff behavior, staffing needs, equipment/supply needs, and opportunities for policy improvement. They monitor staff compliance with OSHA mandates on workplace safety, provide first aid for exposures, and refer employees for post-exposure follow-up. The Supervisor takes job actions with staff members under appropriate circumstances, applies progressive discipline policies, and may remove staff from the unit if necessary to prevent endangerment. All job actions taken are reported promptly to the DON, ADON, and Unit Manager. They also assist the ADON with preparation for long-term care surveys and interact with state surveyors as instructed.
