About The Position

The Stroke Program Coordinator is responsible for collaboratively developing and implementing disease management protocols and guidelines that reflect current research and practices in stroke patient care. This role involves identifying, developing, and participating in hospital and community programs related to stroke. The coordinator will develop and revise stroke care plans and guidelines, communicate these to the multidisciplinary team, and establish clinical benchmarks and quality outcome measures in collaboration with physicians and administrators. Additionally, the position focuses on developing education programs for hospital personnel and the public, ensuring relevant education for patients and families, and assessing the developmental educational needs of staff. The coordinator will conduct ongoing assessments of healthcare personnel's knowledge and clinical skills related to stroke care, serve as a consultant for educational activities, and coordinate stroke care with patients and families during their inpatient stay. This role also involves ensuring appropriate information is available for stroke patient data entry, acting as a liaison between various healthcare professionals, patients, and families to enhance continuity of care, and serving as a resource person for physicians, nursing staff, managers, and ancillary departments. The coordinator will utilize database information to optimize stroke care quality and efficiency, assist in identifying candidates for outpatient follow-up, oversee and coordinate the Survivors of Stroke support group, and ensure comprehensive initial assessments of patients' needs. They will support inpatient continuing coordination of stroke patients' care, coordinate performance improvement efforts for the stroke program, collect and analyze data on stroke care for quality improvement, and present findings at various meetings. Maintaining clinical and professional skills, staying informed about new treatments and therapies, and participating in professional growth activities, including obtaining continuing education on acute stroke patient care, are also key responsibilities. The role includes assisting, coordinating, and participating in Stroke Support groups, assessing group needs, and developing methods to meet them using available resources. Providing ongoing patient/family education, upholding customer service standards, facilitating continuity of care, collaborating with healthcare professionals, and maintaining appropriate patient records are essential. The coordinator will seek physician guidance, act as a liaison for informal marketing of Site Stroke Program services to physicians, identify grant and clinical research opportunities, and assist in identifying appropriate candidates for clinical trials. They will also coordinate strategies to ensure quality, efficacy, and cost-effectiveness of stroke care across multiple settings, maintain up-to-date information on community and health system resources, schedule services and visits efficiently, and serve as a case manager for all stroke patients in collaboration with Care Management to ensure quality care, efficient resource use, and education across the continuum of care. This includes coordinating or completing stroke discharge phone calls.

Requirements

  • Registered Nurse license issued by the state in which the team member practices.
  • CPR
  • NIHSS
  • Bachelor's Degree in Nursing.
  • Typically requires 5 years of experience in ICU, ED or Neurologic nursing, and previous quality improvement experience.

Responsibilities

  • Collaboratively develop and implement disease management protocols/guidelines that reflect the most current research and practices in the stroke patient.
  • Identify, develop, and participate in hospital and community programs related to stroke.
  • Develop or revise stroke written plans of care and guidelines based on accepted standards of care and current research, in collaboration with physicians, nurses and other interdisciplinary professionals.

  • Establish clinical benchmarks and quality outcome measures in collaboration with the stroke Physician Liaison and Administrator for Quality and Clinical Integration.
  • Participate in unit and hospital activities to promote service excellence.
  • Develop education programs on stroke screening, education awareness and treatment options for both hospital personnel and the public.
  • Ensure education for patients, families, and members of the healthcare team is provided and relevant.

  • Identify developmental educational needs of the staff and develop and implement the plan to meet the identified needs, in conjunction with education staff.
  • Conduct ongoing assessments of knowledge and clinical skills of health care personnel caring for stroke patients.
  • Serve as a consultant to unit-based and house wide educational activities.
  • Coordinate stroke care with patients and families during the inpatient episode.
  • Assure appropriate information is available to facilitate patient entry into the Stroke data base.
  • Serve as a liaison between physicians, nursing staff, ancillary staff, patients and families to enhance continuity of care.
  • Serve as an ongoing resource person for physicians, nursing staff, managers and ancillary departments.
  • Utilize data base information to position to offer the best quality and most efficient stroke care to patients.
  • Utilize data base information to analyze quality and efficiency of stroke interventions in stroke patients.
  • Assist in identifying appropriate candidates for outpatient follow-up.
  • Oversee and coordinate the Survivors of Stroke support group.
  • Coordinate and ensure that an initial baseline assessment of patients’ physical, emotional, mental, educational, psychosocial and spiritual needs occurs.
  • Support the inpatient continuing coordination of stroke patients’ care.
  • Coordinate performance improvement efforts for the stroke program.
  • Collect data on stroke care for analysis of the quality of stroke care provided.
  • Review data for quality improvement opportunities and present at Stroke Committee meetings, the Quality Council and other meetings as requested.
  • Provide graphic representation and statistical analysis of data.
  • Maintain clinical and professional skills necessary to provide services to patients.
  • Keep informed of new treatments and therapies utilized for stroke care.
  • Stay abreast of current standards of care via literature, educational events, internal and external resources and internal/external networking.
  • Participate in activities to enhance professional growth by involvement in professional organizations and continuing education.
  • Obtain a minimum of 8 hours of continuing education regarding the care of acute stroke patients.
  • Assist, coordinate, and participate in Stroke Support groups.

  • Assess Group needs and/or desires related to support activities and develop methods to meet needs.
  • Utilize available resources within Advocate Healthcare to address assessed needs of the group (Physician, Nutritional Services, connection to other resources, etc.).
  • Provide ongoing patient/family education, acting as an instructor, counselor, advisor and resource.

  • Seek guidance and direction from the physician based on patient assessment.
  • Act as a liaison and perform informal marketing activities to physicians, outlining services available to their patients through the Site Stroke Program.

  • Assist in identifying appropriate candidates for clinical trials.
  • Work with physicians to identify patients that meet criteria.
  • Provide timely communication of patients’ progress to the physician, as appropriate.
  • Find appropriate grant funding for development of the Stroke Program.
  • Coordinate strategies to ensure quality, efficacy and cost-effectiveness for the stroke patient across multiple settings.
  • Maintain up-to-date information regarding resources that are available in the community and the health system for stroke patients and their families.
  • Schedule services and visits for stroke patients in a timely, cost-effective manner.
  • Serve as a case manager for all stroke patients, in collaboration with Care Management, through an interdisciplinary plan approach to ensure quality care, efficient use of resources and education on the entire continuum of care.
