The Stroke Program Coordinator is responsible for collaboratively developing and implementing disease management protocols and guidelines that reflect current research and practices in stroke patient care. This role involves identifying, developing, and participating in hospital and community programs related to stroke. The coordinator will develop and revise stroke care plans and guidelines, communicate these to the multidisciplinary team, and establish clinical benchmarks and quality outcome measures in collaboration with physicians and administrators. Additionally, the position focuses on developing education programs for hospital personnel and the public, ensuring relevant education for patients and families, and assessing the developmental educational needs of staff. The coordinator will conduct ongoing assessments of healthcare personnel's knowledge and clinical skills related to stroke care, serve as a consultant for educational activities, and coordinate stroke care with patients and families during their inpatient stay. This role also involves ensuring appropriate information is available for stroke patient data entry, acting as a liaison between various healthcare professionals, patients, and families to enhance continuity of care, and serving as a resource person for physicians, nursing staff, managers, and ancillary departments. The coordinator will utilize database information to optimize stroke care quality and efficiency, assist in identifying candidates for outpatient follow-up, oversee and coordinate the Survivors of Stroke support group, and ensure comprehensive initial assessments of patients' needs. They will support inpatient continuing coordination of stroke patients' care, coordinate performance improvement efforts for the stroke program, collect and analyze data on stroke care for quality improvement, and present findings at various meetings. Maintaining clinical and professional skills, staying informed about new treatments and therapies, and participating in professional growth activities, including obtaining continuing education on acute stroke patient care, are also key responsibilities. The role includes assisting, coordinating, and participating in Stroke Support groups, assessing group needs, and developing methods to meet them using available resources. Providing ongoing patient/family education, upholding customer service standards, facilitating continuity of care, collaborating with healthcare professionals, and maintaining appropriate patient records are essential. The coordinator will seek physician guidance, act as a liaison for informal marketing of Site Stroke Program services to physicians, identify grant and clinical research opportunities, and assist in identifying appropriate candidates for clinical trials. They will also coordinate strategies to ensure quality, efficacy, and cost-effectiveness of stroke care across multiple settings, maintain up-to-date information on community and health system resources, schedule services and visits efficiently, and serve as a case manager for all stroke patients in collaboration with Care Management to ensure quality care, efficient resource use, and education across the continuum of care. This includes coordinating or completing stroke discharge phone calls.
