About The Position

The Stroke Navigator plays a pivotal role in delivering seamless, high-quality stroke care from emergency presentation through recovery and secondary prevention. Partnering closely with an incoming Medical Director, this role drives coordinated, patient-centered care, improves outcomes, and strengthens transitions across acute and post-acute settings. You’ll collaborate with interdisciplinary teams, support evidence-based stroke pathways, and elevate the patient and family experience. Requirements

  • Minimum 3 years of clinical nursing experience
  • Experience in stroke care, neurology, ICU, telemetry, or emergency care
  • Strong knowledge of stroke pathophysiology, treatments (including thrombolytics and thrombectomy), and recovery
  • Excellent communication, organizational, and teaching skills
  • Proficiency with electronic medical records and documentation standards
  • NYS Nursing License
  • Valid Driver’s License

Nice To Haves

  • RN with a Bachelor’s Degree
  • Previous inpatient and/or Post Acute care of stroke patients.
  • Previous disease specific navigation experience.

Responsibilities

  • Provide patient-centered navigation for stroke and TIA patients from admission through discharge and post-acute follow-up.
  • Assess patient and caregiver needs, barriers to care, and risks for readmission.
  • Coordinate transitions of care to inpatient rehab, SNF, home health, outpatient therapy, and community resources.
  • Support timely outpatient follow-up with neurology, primary care, and specialty services.
  • Provide individualized education on stroke etiology and warning signs (FAST/BE-FAST), medications and adherence, risk factor modifications and secondary prevention, and lifestyle changes and community resources.
  • Ensure understanding of discharge instructions and warning symptoms requiring urgent care.
  • Support compliance with Joint Commission/DNV/State stroke certification requirements.
  • Participate in stroke quality measures and performance improvement initiatives.
  • Collaborate with stroke coordinators, abstractors, and clinical leadership on outcomes improvement.
  • Serve as a clinical liaison among emergency medicine, neurology, hospitalists, nursing, rehab services, case management, and pharmacy.
  • Participate in stroke rounds, huddles, and multidisciplinary meetings as indicated.
  • Communicate effectively across acute and post-acute settings.
  • Participate in community education, stroke awareness programs, and prevention initiatives.
  • Support relationships post-acute providers, and community partners.
