About The Position

Under the supervision of the HIV Prevention Care Coordination Manager, the Biomedical Prevention Navigator helps clients enroll in HIV biomedical prevention services, including pre-exposure prophylaxis (“PrEP”) and post-exposure prophylaxis (“PEP”) at Men’s Health Foundation’s clinic in West Hollywood. The Prevention Care Coordination team at Men’s Health Foundation works with patients who are at risk of contracting HIV to facilitate access to biomedical prevention services, including assisting with health insurance and providing case management services.

Requirements

  • A bachelor’s degree in public health, social work, psychology, counseling, nursing, human services, or health education OR a minimum of three years conducting outreach to vulnerable populations with the aim of improving health outcomes.
  • Ability to work quickly and accurately to elicit information from patients and enter the information into an electronic database without the patient feeling ignored.
  • Excellent communication and organizational skills; strong written and interpersonal skills
  • Proficient in Microsoft Office (Word, Excel, Access, PowerPoint)

  • Demonstrated ability to provide services to people of diverse races, ethnicities, sexual orientations, gender identities, nationalities, economic backgrounds, religions, ages, English-speaking abilities, immigration status, housing status, and physical abilities in a multicultural environment.
  • Must be able to pass a pre-employment drug test, physical, and a background check to include a 7-year criminal, 10-year SSN & employer history reference check.
  • Must be able to provide proof of COVID-19 vaccination on the first day of work.

  • Must take yearly flu shots or wear flu mask during flu season for patient-facing positions and test for tuberculosis as required by the Centers for Disease Control and Prevention.
  • High School Diploma or equivalent.

Nice To Haves

  • Experience and specific training/experience working with people living with HIV/AIDS preferred.
  • Bilingual in Spanish preferred.

Responsibilities

  • Provide individual counseling/assessments for potential PrEP/PEP clients.
  • Work with clients to assess benefits eligibility and provide PrEP coverage navigation including PrEP-AP enrollment; manufacturer’s assistance program enrollment; and referral to Medi-Cal, Covered California, private plans, or other health insurance coverage for which the client may be eligible.
  • Assist patients with access to supportive services, including mental health treatment, substance abuse treatment, housing assistance, and nutritional support.
  • Document services in Allscripts, MHF’s electronic medical record, county and state databases, as contractually required.
  • Collaborate with team members to identify client needs and generate innovative ideas to engage new patients into PrEP/PEP and other medical services.
  • Conduct PrEP-related presentations and training to staff, patients, and community members in a variety of settings.
  • Attend professional conferences, trainings, and summits to showcase the work of MHF and to gather the latest biomedical prevention trends and data.
  • Identify, coordinate, and participate in community outreach activities in collaboration with biomedical prevention and other MHF teams to increase awareness of PrEP/PEP services and support linkage to biomedical HIV prevention and medical care.
  • Represent MHF in community planning groups, advisory boards and committees.
  • Along with biomedical prevention staff, co-produce outreach and recruitment events to bring visibility to PrEP/PEP program.
  • Participate in all required team, department, and organization meetings.
  • Assist the Benefits Department with enrolling patients in insurance and facilitating provider changes to MHF.
  • Other duties, as required.
