About The Position

The HIV PrEP Navigator provides education, support, and care coordination for patients at risk for HIV, helping them access PrEP (Pre-Exposure Prophylaxis) and related HIV prevention services. This role supports patients in navigating medical, insurance, and social service systems to ensure continuity of care and adherence to PrEP. The HIV PrEP Navigator works closely with clinical teams, behavioral health providers, and community organizations to promote PrEP uptake, retention in care, and harm reduction strategies. This position reports to the HIV Program Manager and plays a key role in reducing barriers to HIV prevention services among high-risk populations. This position is represented by SEIU-UHW. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of SEIU-UHW, as defined in the CBA.

Requirements

  • Bachelor’s degree in Public Health, Social Work, Health Education, or related field OR equivalent experience in HIV prevention, case management, or community health.
  • Minimum of 1 year of experience in PrEP navigation, HIV prevention, or sexual health education.
  • Knowledge of HIV risk factors, PrEP, PEP (Post-Exposure Prophylaxis), and harm reduction approaches.
  • Experience working with LGBTQ+ populations, people experiencing homelessness, or individuals with substance use disorders.
  • Ability to build trust with patients, provide culturally competent care, and address stigma.
  • Strong organizational and documentation skills, including proficiency in Microsoft Office and EHR systems (EPIC preferred).

Nice To Haves

  • Bilingual (Spanish/English) strongly preferred.
  • Familiarity with Medi-Cal, PrEP patient assistance programs, and other healthcare resources.
  • Experience in outreach, group facilitation, or peer education programs.

Responsibilities

  • Conduct patient outreach and education on PrEP, HIV prevention, and sexual health.
  • Identify and engage high-risk individuals, including LGBTQ+ communities, sex workers, individuals with multiple partners, and those with a history of STIs.
  • Assist in PrEP screenings, risk assessments, and linkage to providers.
  • Provide one-on-one counseling to help patients understand PrEP and its benefits.
  • Schedule and coordinate PrEP initiation visits, follow-ups, and lab testing.
  • Assist patients with navigating insurance coverage, Medi-Cal, patient assistance programs, and pharmacy services.
  • Provide reminders and follow-ups to support medication adherence and appointment attendance.
  • Collaborate with behavioral health and case management teams to address social determinants of health impacting PrEP adherence.
  • Build relationships with community organizations, LGBTQ+ groups, and sexual health clinics to expand PrEP access.
  • Represent LifeLong Medical Care in PrEP-related coalitions, events, and outreach programs.
  • Advocate for reducing stigma and increasing awareness of PrEP in underserved communities.
  • Maintain accurate patient records and documentation in the electronic health record (EPIC).
  • Track patient PrEP adherence, follow-ups, and outcomes to report program effectiveness.
  • Ensure compliance with state and federal HIV prevention guidelines.
